MedallionRx is recruiting Consultant Pharmacists on a contract basis for the Indiana and Kentucky markets to join our team. Successful candidates will provide pharmaceutical services to long term care residents in a manner that maximizes quality and patient safety. Pharmacist will manage appropriate drug therapy and review of patient medical charts in accordance with federal and state regulations and MedallionRx policies.
ESSENTIAL FUNCTIONS
* Reviews the drug regimen and clinical indicators of each resident as assigned at least monthly, and reports any irregularities to the Director of Nursing, Administrator and the attending physicians with a written report of findings and recommendations.
* Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability. Makes recommendations for therapy changes to the physician and facility staff as appropriate.
* Documents all communication with members of the healthcare team.
* Has knowledge of pharmacy laws and federal/state regulations regarding LTC.
* Possesses ability to work independently and cooperatively as a member of a team.
* Is knowledgeable of pharmaceutical care practices for long term care residents.
MINIMUM REQUIREMENTS

BS in Pharmacy or PharmD degree, with additional knowledge and/or Certification in geriatrics and/or gerontology

Current pharmacy license in state(s) in which pharmacy practice is conducted

1+ year of pharmacy experience in providing services to long term care centers, or 1+ year Clinical Pharmacy experience in acute, long-term care or home health care.

Proficient in the area of clinical interpretation; drug-drug, drug-disease, drug-food interactions and allergies.

Knowledgeable in the physiology of the aging process and how it affects drug therapy.

Proficiency in the proper interpretation and application of laboratory tests as they relate to the clinical use of drugs.

Previous experience as a Consultant Pharmacist is preferred but not required.
